Started early, took my dog - some spoilers!

Just finished this and I really liked it - although I found it a bit confusing with all the characters.
One thing that I didn't feel was properly answered - whose child was Courteney? I also don't think she ever makes clear who killed Kelly Cross, unless it was supposed to suggest that she was another disposable prostitute.
Anyone help?
