Medium term planning formats not in a linear table? SEN

    Hi, I am a key stage 4 teacher in a sen school. We are currently updating our curriculum and I'm looking for new formats for our medium term plans. Currently they are in a linear table but it just doesn't work as our teaching is more skills based rather than individual subjects (though we still teach english, maths, ICT, art, PSHE, RSE, RE... they are not always seperate units - PE, music and science are taught by specialists) we have on going activities through the term, projects that link different skills areas and EHCP targets, as well as discreet units where needed. We focus on independence, life skills and getting out in the community (leisure, work, shops, facilities, supporting community etc) and our own accreditation modules.
    I would be really interested to see how anyone else goes about this kind of medium term planning or just something to the usual table (or any short term plans in a "different" style too).
