The NQT assessment forms can be used to support your application go through them and identify your areas of strength and the targets you have been set for improvement.Use these in your supporting statement. You can also quote from the successful lesson obs. Don't just rely on an agency for finding posts do apply independently. One thing you should have done is to negotiate the reference before you leave-that is what unions are for.
Teachers have moved on from similar experiences to yourself but a lot depends upon your bounce back and the local job situation in your phase and part of the country.

It can be done across two schools. I did mine across two counties! Like you, the first school I began my NQT year at was dire. I got an interview for the second school, and I 'levelled' with the headteacher and I said what had happened at my first school. I also warned her that my reference was probably not going to be good. She took me on anyway and I passed the NQT year.
It turned out in many ways that the second school was worse than the first, but that is another story. I came away with my NQT year finished and that is all that mattered.
