    If it says "Appropriate degree teaching qualification"., you say BSC (Hons) Chemistry, PGCE (current).
    For "Proven ability to teach subject X with substantiated impact on achievement ", put Observations on teaching practice show good teaching and all pupils learning. Or similar, but has to be true of course!
    You have to omit some, to keep to one page and because too much is too much. Leave out any not relevant to you as a student. Then choose the most important - not the desirable clearly.
