Dear Theo.."Please complete form in black ink"

    Further to those quite rightly advising that 'black ink' does include typing it, the instruction is meant for those who might occasionally fill out the form in blue biro or - and those asking 'how can I make my form stand out?' should pay attention here - red or green biro.
