Normally means the Head, or PGCE mentor. You could put your supply agency but they always give very brief references. Is there no school where you have gone back more than once? How about someone there? Q3 Send it as a separate document. If you are sending a full application letter, then put the E.S. as the third page of the letter, so it gets printed out automatically. Sometimes there is a whole page in the application form called something like Additional information. In that case put the E.S. there and then have a full letter of application.
